[*] Scope of Service:

(1) Vendor needs to provide afterhours call out service for facilities maintenance to perform after-hours call-out response services for buildings, building systems and building components.
- The requested after- hours call-out services are 5:00 p.m. to 8:00 a.m. Monday through Friday, and 24 hours a day on weekends and holidays.
- The first responsibility of the Bidder is to deploy one person to respond to the facility and investigate the issue.
- The second responsibility of the Bidder is to determine the necessary action, i.e., if an inspection is necessary, example: an alarm was triggered, then an inspection is required and possibly the resetting of the alarm if required; or if there is actual physical damage that requires repair to the facility, examples: water leak or broken lock.
- The third responsibility is to determine if the respondent on site can mitigate the situation alone, if additional staff and supplies are required to mitigate and contain the issue, or if the issue requires an emergency response, i.e., fire or police for an emergent health and safety situation.
- The successful bidder shall provide a 24-hour contact phone number to the project manager and the communication system.
- Project Manager for all call-out responses. The report shall include the date of the call-out, the requestor’s name and facility, date and time call was received, date and time Bidder arrived at the facility, nature of the call- out and the resulting action taken. The report shall be in written or electronic form and shall be submitted to the FNSB Project Manager or his designee within 24 hours of the call-out.
(2) All the questions must be submitted no later than May 12, 2023
