[*] Scope of Service:

(1) Vendor needs to provide telephone triage nursing services for advice to staff caring for the individuals residing at community residences.
- able to provide service Monday through Thursday between the hours of 4:30 p.m. to 8:30 a.m. and from Friday at 4:30 p.m. through Monday at 8:30 a.m., and 24 hours on Observed Holidays
- provide Triage Services to individuals based on: verbal discussions with staff; the individuals’ medical history
- Review of Calls: the Contractor must provide, upon request, audio recordings or written transcripts of calls or a mechanism to review calls or access to speak with RN who handled call
- Contractor will be responsible for tracking and billing for services provided monthly. This shall include, but not be limited to, a monthly aggregate report listing calls received, staff person calling, individual’s name and address, and a brief description of the call itself.
- Payment will be based on the monthly admin fee plus rate per call.
(2) Approximately call volume will be 190 calls per month.
(3) All questions must be submitted no later than April 28, 2023.
