[*] Scope of Service:

(1) Vendor needs to provide telephone triage nursing services.
- Provides telephone triage nursing services to approximately 660 individuals living in approximately 145 homes.
- Telephone triage nursing service is needed to provide advice to staff caring for the individuals residing.
- Telephone triage nursing is a service by which a registered nurse screens an individual’s symptoms based on the information provided during the call.
- The anticipated usage averages approximately 200 calls per month.
- Provide service Monday through Thursday between the hours of 4:30 p.m. to 8:30 a.m. and from Friday at 4:30 p.m. through Monday at 8:30 a.m.,
- Provide Triage Services to individuals based on: verbal discussions with agency staff, the individuals’ medical history, when available; and the policies, practices, and protocols mutually developed and agreed upon.
- Provide, upon request, audio recordings or written transcripts of calls or a mechanism to review calls or access to speak with registered nurse who took call.
- Responsible for tracking and billing for services provided monthly
- Payment will be based on the monthly admin fee plus rate per call.
- Providing recommendations based on established clinical protocols and nursing assessment
(2) All questions must be submitted no later than March 28, 2023.
(3) The contract period will be for five years.
