[A] Budget: Looking for Proposal

[B] Scope of Service:

(1) Vendor needs to provide call center service.
- Provide supportive services to help frail seniors and people with disabilities remain independent and live outside of institutions
- Service is provided under a federal Medicaid waiver and is specific in determining that transportation only occurs from home to work sites, workshops, or training centers]
- Provide urgent, short-term assistance to medical appointments for Veterans who are not eligible or unable
- Direct customers in emergent need to the appropriate emergency resources. The Contractor shall refer emergency calls to 911 as necessary
- Call Center programs, additional contracts represent different levels of service and expectations as stipulated in the different service agreements.
- Capability of managing a variety of reporting and invoicing requirements in a variety of formats
- Clients from other funding agencies may access transportation services through the Call Center if they are eligible according to the requirements established by those funding agencies.
- Confirm eligibility status for all programs. Contractor staff shall assess the need for services; determine the appropriate transportation needs of the rider; select the least cost, most appropriate alternative; and arrange for the service to be provided from a wide pool of modes and subcontracted transportation operators
- To conduct eligibility assessments with the goal of developing an integrated mobility management system that provides access for all of the transportation services offered.
- Provide information to potential riders about the eligibility process. Requests for services may come from any source – the potential rider, friend, relative, advocate
- Taking ride requests
- Verifying eligibility
- Verifying medical appointments
- Arranging transport with providers
- Entering ride and customer-related data on computer
- Generating reports and billings
- Verifying provider billings
- Sending out client notifications
- Distributing bus passes and tickets
- Requesting client has no other available means of transportation to medical services.
(2) A Pre-Proposal Meeting will be held on August 26, 2022.
(3) All question must be submitted no later than September 13, 2022.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
