[A] Budget: Looking for Proposal

[B] Scope of Service:

(1) Vendor needs to provide domestic outreach triage services.
- triage coordinator will conduct a minimum bi-weekly query of all 911 calls coded as domestic violence incidents in the targeted “hot spot” areas in 3rd, 4th and 5th precincts where a police report was not generated.
- triage coordinator will gather background information related to the call and the household through various law enforcement and court systems, to include: MNCIS, MGA, PIMS (CAPRS), VISINET and DOCCR to determine the potential resource needs of the household.
- the triage coordinator will create a referral form for each call and make appropriate referrals to the MPD community navigators and community-based resources to address the specific needs of the household.
- the triage coordinator will also track the referrals, seek feedback, and adjust the process and information gathered as necessary
- the triage coordinator will conduct resource investigation and mapping to
determine what is available and what may need to be created or expanded.
- as a member of the core project development group, the triage coordinator will assist with program development, expansion and innovation, including ongoing evaluation of the project model, tracking forms utilized and service delivery process from triage to referral
- the triage coordinator will attend and assist with facilitation of all project meetings as needed
- the triage coordinator may be expected to conduct trainings and presentations as they arise around the scope of the project, the work of the coordinator and other project related topics as needed
- as the program evolves, there may be other duties that arise which will fall to the responsibility of the triage coordinator, these will be completed and tracked for future evaluation and evolution of the consultant’s requirements.
(2) A pre-proposal conference will be held on January 31, 2020.
(3) All questions must be submitted no later than February 4, 2020.
(4) The term of the contract will be three years.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
