[B] Scope of Service:

(1) Vendor needs to provide hotline and single point of entry services
- The contractor will maintain an ongoing call log and an inquiry and crisis response form on all calls and will send a secure (encrypted) electronic record of these completed forms to the center by 10:00 a.m. the following business day,
- The Contractor will maintain an ongoing call log on all calls and will send this log via electronic transmission twice a day 8:00 a. m. and 3:00 p.m. for all records after last transmission,

- Contractor staff will identify the nature of the presenting problem and the service/support needs of the caller, the caller's safety and clinical issues are the top priority,
- Telephone intervention is specific to the callers' clinical needs and functioning,
- Demographic information of the caller is obtained for the purpose of identification and follow-up, the caller will be triaged to the least restrictive care environment,
- Contractor's staff will immediately notify on-call staff of the center of all calls which are emergent or urgent in nature and require intervention,
- The contractor's staff will also initiate contact with local EMS and or law enforcement agencies specific to the area from where the person is calling.
(2) All the questions must be submitted no later than November 13, 2019.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
